Where is North Shore Rheumatology Located?
We are located on the campus of Northwestern Lake Forest Hospital in the 900 N Westmoreland Building, Suite 218
Where should I park?
Parking for the office is available across the street from the 900 building. If you are having trouble finding parking or would prefer, there is complimentary Valet Parking available at the front entrance of the 900 building.
Should I fill out and bring the patient forms with me to my appointment?
It is required to have your patient forms completed prior to your appointment arrival. We have provided most of the forms you need in our patient forms section of the website. You can access the forms which are in pdf format through that page. Simply fill them out and bring them with you at your appointment.
Please bring with you a photo ID, and your insurance card. Also be prepared to pay any copayment that your insurance states needs to be paid prior to seeing your provider.
Can I email you with questions regarding my care?
No, Our portal is for general inquiries only. Non-emergent inquiries can also be made through your patient portal
What are you hours?
Our hours are below. If you need medical attention when our office is closed, please contact us as 847.739.2126. If it is a life threatening situation, please call 911.
